I hereby grant End in Mind Project the irrevocable right and permission to use my submitted story, photographs and/or audio recordings on End in Mind Project and other websites and in broadcasts, publications, promotional flyers, educational materials, derivative works, or for any other similar purpose.

I understand and agree that such stories, photographs and/or audio recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the words, photographs and/or audio recordings of me. I waive the right to approve the final product. I agree that all such stories, photographs and/or audio recordings, and any reproductions thereof, and all digital files are and shall remain the property of End in Mind Project. I also hereby certify that the information I am submitting is my own personal story. I further certify that I am not submitting information, photos, videos, or audio recordings that belong to anyone else known or unknown to me.
This release is binding on me and my personal representatives.
